Description
Anterior to the distal femur and proximal tibia, the patella sits within the extensor mechanism, its medial border tethered to the medial patellofemoral ligament (MPFL) as it courses from the medial femoral epicondylar region toward the superomedial patella. Medial and lateral femoral condyles frame the patellofemoral articulation, while the tibial plateau lies inferior, capped by articular cartilage. The fibular head appears lateral to the proximal tibia, helping orient laterality in this anterior knee view. Clear landmarks. Medial patellofemoral ligament anatomy matters because the MPFL provides the dominant soft tissue checkrein to lateral patellar translation in early knee flexion, and it is the structure most often disrupted after a traumatic lateral patellar dislocation. Getting the femoral attachment right, close to the medial epicondyle and adjacent to the adductor tubercle region, is the make-or-break point in MPFL reconstruction; malposition shifts graft isometry and can drive medial patellar overload, anterior knee pain, and accelerated chondral wear on the medial patellar facet.
