Description
Arising from the medial expansions of the quadriceps tendon and blending with the medial patellar ligament, the medial patellar retinaculum forms a thin fascial sheet spanning from the medial border of the patella toward the medial femoral epicondyle and proximal tibia. From a medial perspective, the patella sits anterior to the femoral trochlea, while the tibial plateau lies inferior, with the fibular head remaining lateral and only partly in view. Deep to the retinaculum, the medial patellofemoral ligament is typically appreciated as a reinforcing band running from the superomedial patella to the adductor tubercle region. Soft tissue planes matter here. Medial restraint of the patella is a daily clinical problem, not an academic one. Lateral patellar dislocation commonly injures the medial patellofemoral ligament and adjacent medial patellar retinaculum, producing medial-sided tenderness and hemarthrosis, and predisposing to recurrent instability if maltracking drivers (trochlear dysplasia, patella alta, increased TT-TG distance) persist. For surgeons performing MPFL reconstruction or medial retinacular imbrication, this view clarifies the relationship between the patellar medial border, the femoral attachment near the medial epicondyle, and the tibial fascial expansions that can be overtightened, leading to medial patellar overload.
