Description
Arising from the common tendinous ring (annulus of Zinn) at the orbital apex, the medial rectus muscle courses anteriorly along the medial wall of the orbit to insert on the sclera just anterior to the equator. Its muscle belly remains parallel to the globe, lying medial to the optic nerve and opposite the lateral rectus across the transverse axis of the eyeball. A short, pale tendon caps the anterior end of the muscle at its scleral footprint. Portions of the superior rectus and the superior oblique tendon can be appreciated superiorly as neighboring landmarks. Medial rectus anatomy matters any time you are thinking about horizontal ocular alignment. Isolated weakness from an oculomotor nerve (CN III) palsy leaves the lateral rectus unopposed, classically producing an abducted eye with diplopia that worsens on attempted adduction. For strabismus surgery, the medial rectus is the workhorse muscle for recession and resection procedures; its predictable insertion and straight-line pull make it a standard reference when teaching extraocular muscle actions and their clinical testing. Use this asset in ophthalmology and neuroanatomy teaching to correlate extraocular muscle actions with cranial nerve lesions, and in surgical atlases or patient education materials covering medial rectus recession for esotropia. It also fits well in head and neck anatomy courses that introduce the orbital apex and the annulus of Zinn as a shared origin for the recti.
