Description
Framed in profile, the adult male globe sits centrally with the cornea anterior and the sclera forming the outer coat, while the inferior rectus muscle runs along the inferior aspect of the eyeball from a posterior origin toward its anterior scleral insertion. Its tendon is shown blending with the fascial sheath of the eyeball (Tenon’s capsule) just posterior to the limbus, with the muscle belly lying inferior to the visual axis and the inferior oblique coursing more obliquely beneath it. Medial and lateral rectus muscles bracket the globe on either side, and the superior rectus occupies the superior pole, creating a clear sense of how the recti form a muscular cone around the optic nerve as it exits posteriorly. Isolating the inferior rectus in this relationship to Tenon’s capsule matters in strabismus surgery, where recession or resection depends on accurate placement at the scleral insertion and careful dissection through conjunctiva and Tenon’s to avoid scleral perforation. The inferior rectus is also the extraocular muscle most often restricted in thyroid eye disease, producing hypotropia and limited elevation (a positive forced duction test), and it can be secondarily trapped in orbital floor blowout fractures with diplopia on upgaze. Tendon blending into the fascial sheath is the surgical plane. It is also the tether.
