Description
Seen in a lateral orbital profile, the lateral rectus muscle runs anteriorly from the common tendinous ring at the orbital apex to its insertion on the temporal sclera, lying lateral to the optic nerve as it courses posteriorly from the globe. Superior and inferior rectus muscles occupy the superior and inferior aspects of the bulb, while the medial rectus tracks along the nasal side, balancing the lateral rectus across the coronal plane of the eye. Oblique muscles may be partially visible as they angle across the globe, contrasting with the straighter fiber direction of the recti. Fiber striations emphasize the longitudinal orientation of the extraocular musculature. Abduction of the eye depends on the lateral rectus, and this view makes its relationship to the optic nerve and posterior orbit easy to teach. It is the muscle classically affected in abducens nerve (CN VI) palsy, producing an esotropia that worsens on gaze toward the affected side and often follows raised intracranial pressure or cavernous sinus pathology. Surgeons also reference the lateral rectus insertion during strabismus recession or resection, where millimeters matter and the nearby lateral palpebral structures and orbital fat can limit exposure.
