Description
Arising from the common tendinous ring at the orbital apex, the inferior rectus muscle runs anteriorly along the inferior aspect of the globe and inserts onto the sclera anterior to the equator, opposite the superior rectus. The medial rectus lies nasally (medial) and the lateral rectus temporally (lateral), forming a rectus cone around the optic nerve as it exits the posterior pole. Superior and inferior oblique muscles course obliquely relative to the recti, with the inferior oblique sweeping from the anteromedial orbital floor toward the posterolateral globe. Fine episcleral vessels trace the scleral surface near the insertions. Clear landmarks. Functionally, the inferior rectus is the primary depressor of the adducted eye and a contributor to extorsion, so its line of pull matters when teaching ocular motility testing and interpreting diplopia patterns. Restriction or tethering at the inferior rectus is a classic mechanism in thyroid eye disease, producing limited upgaze and vertical diplopia that worsens in elevation, while iatrogenic injury or scarring after inferior rectus recession can create postoperative hypotropia. The proximity of the rectus insertions to the anterior segment also makes this anatomy relevant for strabismus surgery planning, including safe dissection planes around Tenon’s capsule and awareness of anterior ciliary vessels traveling with the recti.
