Description
Medial segment (segmentum mediale, S5) of the right middle lobe is presented in profile, oriented on the mediastinal surface where the lung abuts the pericardium and right atrium. Superior to it lies the horizontal fissure separating the middle from the superior lobe, while the oblique fissure runs posteroinferiorly toward the lower lobe. The segment’s medial face is directed toward the cardiac impression, with its lateral aspect continuous with the adjacent lateral segment (S4). Bronchovascular structures logically converge centrally at the segmental hilum. This segment matters because it sits against the heart and mediastinum, so pathology here can mimic cardiac disease or produce subtle radiographic signs on frontal views. Middle lobe disease often presents as right middle lobe syndrome, where recurrent atelectasis or infection follows obstruction or impaired collateral ventilation, and correct segmental localization guides bronchoscopy and targeted physiotherapy. Segmental anatomy also underpins parenchyma-sparing surgery, since a right middle lobectomy is not always necessary when an isolated S5 lesion can be addressed with segmentectomy along intersegmental planes.
