Description
Rendered in lateral profile, the medial semispinalis capitis is exposed as a deep posterior cervical muscle coursing superomedially from the upper thoracic and lower cervical region toward its insertion on the occiput between the superior and inferior nuchal lines. Superficial layers such as trapezius and splenius capitis are displaced or removed, leaving the semispinalis capitis identifiable by its long, vertically oriented fiber bundles that sit posterior to the cervical vertebrae and medial to longissimus capitis. Anteriorly, the sternocleidomastoid and masticatory muscles including temporalis and masseter remain visible as orientation landmarks around the mandible and temporal fossa. Depth reads clearly. Semispinalis capitis is a prime generator of head and neck extension and contralateral rotation, and its medial fibers matter in the clinic because they are commonly involved in cervicogenic headache patterns where pain refers to the occipital region. Proximity to the greater occipital nerve as it emerges in the posterior scalp makes this layer relevant when discussing occipital neuralgia, trigger-point injection targets, and postoperative pain after posterior cervical approaches that split or retract the extensor mass. This is a muscle you can miss if you only learn the superficial neck.
