Description
Along the medial plantar margin of the male foot, the abductor hallucis forms a superficial muscular band running anteroinferiorly from the calcaneal tuberosity toward the hallux. Its belly lies inferior to the medial malleolus region and parallels the medial longitudinal arch, with the plantar fascia superficial and the deeper plantar musculature positioned lateral and deep to it. Distally, the muscle narrows into a tendon that courses toward the medial side of the first metatarsophalangeal joint to blend with the medial sesamoid complex and the base of the proximal phalanx of the great toe. Medial exposure of abductor hallucis matters because this muscle defines the medial boundary of the tarsal tunnel region and sits adjacent to the neurovascular bundle as the tibial nerve divides into the medial and lateral plantar nerves beneath the flexor retinaculum. This is the corridor where tarsal tunnel syndrome pain can be reproduced and where surgical release must respect the relationship between the abductor hallucis fascia, the calcaneus, and the branching pattern of the plantar nerves.
