Description
Centered in the anterior abdomen, the large intestine forms a broad frame with the cecum and vermiform appendix in the right lower quadrant, the ascending colon coursing superiorly to the hepatic flexure, the transverse colon crossing to the splenic flexure, and the descending and sigmoid colon descending to the rectum. Fine mesenteric nerve branches fan out within the mesocolon, tracking along the arterial arcades and vasa recta supplying each colonic segment. Discrete ganglionic enlargements are suggested at branch points, consistent with autonomic plexus organization as fibers travel from periarterial plexuses into the bowel wall. Haustra and appendices epiploicae mark the colonic surface, while the neural fascicles remain deep, posterior to the serosa and aligned with the mesenteric attachments. An anterior perspective like this clarifies how sympathetic and parasympathetic fibers distribute to the hindgut through the inferior mesenteric plexus and along the inferior mesenteric artery and its branches, a pattern that becomes clinically concrete during left colectomy and low anterior resection. Small nerves run with the vessels. That relationship explains why high ligation near the origin of the inferior mesenteric artery risks autonomic injury, contributing to postoperative constipation, altered rectal compliance, or sexual and bladder dysfunction when hypogastric plexus pathways are disturbed. For teaching, it also helps distinguish extrinsic autonomic supply in the mesentery from the intrinsic enteric plexuses embedded in the muscularis propria.
