Description
Running distally from the carpus to the bases of the proximal phalanges, the five metacarpals (I to V) are highlighted in blue against the remaining hand skeleton. Metacarpal I sits most lateral on the radial (thumb) side and is shorter and more mobile, while metacarpals II and III align more centrally and form a stiffer longitudinal column; metacarpals IV and V lie ulnarly and show a more pronounced palmar concavity across their shafts. Proximally, each metacarpal articulates with the distal carpal row at its base, and distally each head forms the metacarpophalangeal joint just proximal to the proximal phalanx. Clean landmarks. Anterior (palmar) presentation is the teaching workhorse for metacarpal orientation because it makes the transverse metacarpal arch and the relative rotation of the metacarpal heads easier to appreciate than a dorsal view. That matters in the clinic: neck fractures of the fifth metacarpal (boxer’s fractures) typically angulate with apex dorsal, and a clear palmar reference helps explain why knuckle contour changes and why reduction aims to restore metacarpal length and rotational alignment to prevent digital scissoring.
