Description

Peripherally, periosteum overlies the compact cortex, while deeper bone transitions to trabecular (cancellous) architecture with marrow spaces, highlighting the abrupt shift from dense lamellar bone to a lattice of trabeculae. Transverse Volkmann canals perforate the lamellar rings and connect adjacent Haversian canals, running perpendicular or oblique to the long axis of the osteon. Orientation of Haversian versus Volkmann canals is the teaching point. That relationship explains how cortical bone remains viable despite its density, and why disruption of intracortical blood flow matters clinically: intracapsular femoral neck fractures can compromise retinacular vessels and predispose to osteonecrosis, while periosteal stripping in open fractures or aggressive surgical exposure can impair cortical perfusion and delay union. Remodeling also reads cleanly in this view, since osteons represent the endpoint of osteoclastic tunneling and osteoblastic infilling; hyperactive turnover in osteoporosis or osteomalacia is easier to conceptualize when osteocyte lacunae and vascular channels are shown together.
