Popliteus Muscle

Description

Anteriorly, the distal femur descends to the knee where the proximal tibia and fibula frame the joint line, with the patellar region implied by the femoral condyles even though soft tissues are stripped away. A single muscle is isolated in red, the popliteus, seen deep to where the posterior capsule would lie, spanning obliquely from the lateral side of the distal femur toward a medial, inferior attachment on the posterior proximal tibia. Its triangular belly sits just inferior to the lateral femoral condyle and angles medially across the back of the joint. Bony relationships stay explicit: femur superior, tibia inferior and medial, fibula inferior and lateral. Popliteus anatomy matters because its tendon passes intra-articularly (extrasynovial) and relates closely to the lateral meniscus, so tears of the posterolateral corner can destabilize both rotation and meniscal motion. Clinically, this muscle is discussed when teaching the screw-home mechanism and the initiation of knee flexion, where popliteus contributes to internal rotation of the tibia in open-chain movement and external rotation of the femur in weight-bearing. Small structure, big diagnostic value. Orthopaedic and sports medicine educators can place this plate alongside models of ACL injury to explain why posterolateral corner disruption produces abnormal external tibial rotation and varus opening, and why tenderness near the posterolateral joint line may implicate popliteus tendon pathology.
