Description
Oblique lateral anatomy centers on the orbit and upper eyelid, with the levator palpebrae superioris coursing anteriorly from the posterior orbit toward the superior tarsal plate. Its tendon expands as the levator aponeurosis, lying deep to the orbital septum and preaponeurotic (orbital) fat, then fanning into the upper lid to blend with the tarsal plate and overlying skin. Superiorly and laterally, the lacrimal gland occupies the lacrimal fossa of the frontal bone, while the bony margins of the orbit, the frontal, parietal, and occipital bones, and the zygomatic and maxillary components frame the facial skeleton. Levator morphology matters because small differences in aponeurosis insertion and its relationship to orbital fat drive the clinical appearance of the upper lid crease and the pattern of eyelid ptosis. This perspective maps the anatomic corridor used in upper eyelid surgery, where the surgeon works between orbicularis oculi, orbital septum, and preaponeurotic fat to identify and advance the levator aponeurosis, and it clarifies how disinsertion after aging or trauma produces aponeurotic ptosis with a high lid crease.
