Description
Running deep to the sternocleidomastoid, the upper deep cervical lymphatics form a vertical chain of lymph nodes and vessels paralleling the internal jugular vein from just inferior to the angle of the mandible toward the carotid sheath. Superiorly, nodes cluster near the jugulodigastric region beneath the posterior belly of the digastric and adjacent to the parotid tail, with efferent vessels coursing inferiorly toward the jugulo-omohyoid level. The relationship to the carotid artery and vagus nerve is implied by their shared fascial corridor, while superficial facial muscles, the mandible, and the external ear provide lateral surface landmarks. Nerves of the cervical plexus and cranial nerve branches traverse the field, emphasizing how closely lymphatic channels thread between neurovascular structures. For head and neck clinicians, this chain is the first-echelon drainage for the oropharynx, posterior nasal cavity, and portions of the oral cavity, and it is a common site of metastatic spread from squamous cell carcinoma. Palpable enlargement at the upper internal jugular nodes often tracks with tonsillar or base-of-tongue primaries, and the jugulodigastric node remains a practical clinical landmark during examination. Surgeons working in the lateral neck must respect the adjacency to the spinal accessory nerve in the upper neck and the internal jugular vein itself during selective neck dissection or lymph node biopsy. Small distances matter.
