Description
Horseshoe kidney anatomy centers on fusion of the inferior (lower) poles of the right and left kidneys across the midline, forming an isthmus that bridges the two renal masses. Each renal hilum sits on the medial border, with the renal pelvis and proximal ureter oriented anteromedially compared with typical adult rotation, and the ureters course inferiorly toward the bladder while crossing the anterior surface of the isthmus. The fused complex lies lower than usual in the abdomen, with the isthmus positioned anterior to the great vessels, a relationship that becomes obvious in a fixed anterior-facing composition. No two are identical. Clinically, this congenital anomaly matters because renal ascent and rotation are altered, which explains the frequent association with ureteropelvic junction (UPJ) obstruction, hydronephrosis, and recurrent urinary tract infection.
