Description
Kidney parenchyma dominates the frame, with a deep laceration tracking from the outer renal cortex into the underlying medulla, disrupting the normal corticomedullary architecture. Along the convex lateral border, the fibrous renal capsule appears breached where the tear gapes, and a localized hemorrhage stains the cortical surface adjacent to the defect. Medially, the concave hilum region provides orientation for the renal sinus, where the renal pelvis would normally funnel urine toward the proximal ureter, even if the collecting system itself is only partly exposed from this fixed viewpoint. Blood pools on the capsular surface. Blunt abdominal trauma from a motor vehicle collision or a direct flank impact produces this morphology, and the illustration maps well to higher grade renal injury where a parenchymal laceration raises concern for urinary extravasation, segmental vascular disruption, or a developing perirenal hematoma. By keeping attention on the cortical breach and the depth of the parenchymal tear, the image supports correlation with contrast-enhanced CT findings, such as a deep cleft extending to the renal sinus or associated perinephric stranding, that guide observation versus intervention. The focal hemorrhage pattern also helps teach why macroscopic hematuria can be absent despite substantial parenchymal injury, depending on whether the collecting system is violated.
