Rendered in a sagittal cross-section, the uterus is opened to expose the endometrial cavity centrally, the surrounding myometrium, and the outer serosal contour where a subserosal uterine fibroid (leiomyoma, myoma) projects from the fundal or lateral wall. Inferiorly, the cervix forms the narrowed transition into the vaginal canal, with the cervical canal aligned to the uterine cavity. Lateral to the uterine body, an ovary with follicular structures sits adjacent to a partially seen uterine tube approaching the uterine cornu. The mass remains external to the endometrium, displacing the serosal surface outward rather than bulging into the cavity. Subserosal fibroids matter because their symptoms and management differ from submucosal lesions: they more often cause bulk-related complaints (pelvic pressure, urinary frequency, constipation) than heavy menstrual bleeding, since the endometrial interface is usually preserved. Orientation to the serosa and myometrium is also what guides procedure selection, since laparoscopic or robotic myomectomy targets the serosal surface while hysteroscopic techniques are reserved for intracavitary or submucosal disease. Pedunculated subserosal myomas can twist on their stalk, a specific cause of acute pelvic pain with focal tenderness and sometimes a red degeneration pattern on MRI. That distinction is the teaching point.
