Description
Arising from the external occipital protuberance and median nuchal line, the nuchal ligament forms a thick midline septum that descends to the spinous process of C7 in a lateral profile of the male skull and neck. Posterior to the cervical vertebrae, its sheet-like fibers blend with the deep cervical fascia and provide a tendinous partition between the right and left posterior neck musculature. Anterior and inferior to this, the mandible with dentition and the maxilla define the facial skeleton, while the hyoid bone sits suspended in the anterior neck, deep to the mandibular body. The auricle and external acoustic meatus establish an otic landmark just lateral to the mastoid region. Clinically, this view helps orient the posterior cervical midline, where the nuchal ligament is used as a palpable landmark during neuraxial access at the cervicothoracic junction and for consistent midline identification in posterior approaches to the cervical spine. Pain generators in whiplash and chronic neck strain often localize near ligamentous and fascial attachments at the occiput and C7, and the nuchal ligament also defines a dissection plane when reflecting trapezius and splenius capitis in posterior cervical exposure. A clean midline matters.
