Description

Arising from the flexor retinaculum and the trapezium on the radial side of the carpus, the opponens pollicis fans distally and laterally to insert along the lateral border of the first metacarpal shaft, deep to abductor pollicis brevis and flexor pollicis brevis in the thenar eminence. From an anterior (palmar) perspective, the thenar muscles occupy the radial, superficial palm while the central palmar aponeurosis and the flexor digitorum superficialis and profundus tendons track distally toward the digits in the midline. The digital flexor tendon sheaths and transverse carpal ligament (flexor retinaculum) form the proximal boundary at the wrist crease, with the carpal bones deep to this plane. Median nerve territory. Opponens pollicis is the workhorse for true thumb opposition, producing pronation and flexion of the first metacarpal at the carpometacarpal joint, and this view clarifies why loss of thenar function translates into impaired pulp-to-pulp pinch rather than a simple decrease in thumb flexion. Clinically, the muscle becomes weak or atrophic with median nerve compression in carpal tunnel syndrome or proximal median neuropathy, and it is a key target when distinguishing a low median palsy from an ulnar neuropathy during a focused hand exam.
