Description
Posterior male pelvic anatomy is presented with the sacrum and coccyx centered, flanked by the posterior ilium and the ischial tuberosities, with proximal femora partially included inferiorly. From the posterolateral pelvis, the posterior cutaneous nerve of the thigh gives off its perineal branch, which sweeps anteromedially toward the perineum and external genital region. Superficial sensory territory is implied rather than fully mapped, but the course places the perineal branch medial to the gluteal region and directed toward the posterior scrotal and perineal skin. Fine caliber, long trajectory. Clinically, this view helps separate posterior femoral cutaneous nerve contributions to perineal sensation from the pudendal nerve, a distinction that matters when evaluating posterior scrotal or perineal numbness after prolonged cycling, pelvic trauma, or regional anesthesia. Because the nerve arcs forward from a posterior origin, it is vulnerable to traction or compression near the ischial tuberosity and along the gluteal fold, and it can be mistaken for pudendal entrapment when symptoms localize to the perineum. The posterior perspective also supports teaching the relationship between sensory complaints and the bony landmarks used for injections, nerve blocks, and surgical positioning.
