Description
Arising from the lateral pontine and cerebellopontine angle region, the petrosal vein (superior petrosal vein) is traced from an inferior perspective as it courses a short distance along the petrous part of the temporal bone to drain into the superior petrosal sinus. Nearby, the undersurface of the brainstem (pons and medulla) sits medial to the petrous ridge, with the cerebellar hemisphere and flocculus positioned posterior and inferior to the venous channel. Cranial nerves emerging from the ventral brainstem, including the trigeminal nerve near the mid-pons and the facial and vestibulocochlear nerves at the pontomedullary junction, provide tight neurovascular neighbors in this corridor. Relationships are compact. That compactness is the point clinically. The petrosal venous complex is a frequent constraint during microsurgical approaches to the cerebellopontine angle, including vestibular schwannoma and meningioma resections, where sacrificing the superior petrosal vein can precipitate venous infarction or cerebellar edema in select patients. Endovascular and skull base teaching also benefits from seeing how a relatively direct venous tributary feeds a dural sinus at the petrous ridge, helping you anticipate where venous bleeding occurs when working near the tentorial edge and Meckel’s cave.
