Description
Arising from the anterior surface of the sacrum, the piriformis is rendered in profile as it courses laterally through the greater sciatic foramen toward its insertion on the superior border of the greater trochanter. Superior and posterior to the acetabulum, its belly lies deep to the gluteal musculature and spans the posterior aspect of the hip joint capsule. Bony landmarks of the pelvis and proximal femur frame the muscle, and the lateral hip view also exposes the femoral head and neck relationship to the acetabulum. A total hip arthroplasty is present, with metallic acetabular and femoral components seated within the pelvis and proximal femur. Clinically, this angle clarifies how a small deep external rotator can influence symptoms that patients localize to the buttock and posterior hip, and it supports teaching around piriformis-related sciatic nerve irritation as the nerve exits inferior (more commonly) or occasionally through the muscle.
