Description
Seen from the lateral hip, the piriformis muscle spans from the anterior surface of the sacrum to the superior border of the greater trochanter, coursing laterally through the greater sciatic notch to exit the pelvis via the greater sciatic foramen. The posterior ilium, acetabulum, and proximal femur frame the muscle’s oblique trajectory, with the femoral head seated medially within the acetabulum and the greater trochanter projecting laterally as the piriformis insertion landmark. Superiorly, the lower lumbar vertebrae and lumbosacral junction provide bony context for the sacral origin. A clear muscular focal point. This lateral perspective matters because piriformis is a practical surface-to-depth landmark for the deep gluteal region, dividing the greater sciatic foramen into a suprapiriform and infrapiriform compartment and predicting the expected course of the superior and inferior gluteal neurovascular bundles.
