Deep to the plantar aponeurosis, stout plantar tarsometatarsal ligaments bridge the medial cuneiform to the base of the second metatarsal and continue laterally across the cuneiforms and cuboid, forming dense bands that run generally anterodistal while resisting separation of the metatarsal bases. Bony landmarks likely visible include the calcaneus posteriorly, the cuboid laterally, the three cuneiforms medially and centrally, and the metatarsal shafts extending distally toward the proximal phalanges. Stability here is not cosmetic anatomy, it is load transfer. The plantar ligamentous layer is the primary restraint against dorsal displacement at the tarsometatarsal joints, which is why Lisfranc injuries often present with plantar ecchymosis and why subtle diastasis between the first and second metatarsal bases can signal failure of the Lisfranc ligament complex.
