Description
Plantar anatomy is centered on the sustentaculum tali of the calcaneus and the plantar calcaneonavicular (spring) ligament spanning anteriorly and medially to the plantar surface of the navicular, directly inferior to the head of the talus. From this inferior perspective, the talus sits superior to the ligament, while the calcaneus lies posterior and lateral and the navicular lies anterior and medial, forming the medial column of the foot. Nearby, the long plantar ligament and plantar fascia would lie more superficial and longitudinal, while the tendon of tibialis posterior courses distally to insert primarily on the navicular tuberosity and expansions to the cuneiforms. Functionally, the spring ligament forms the fibrocartilaginous floor of the talonavicular joint and resists plantar and medial drift of the talar head, so it is a primary static support of the medial longitudinal arch. Failure of this support commonly appears in adult acquired flatfoot, where tibialis posterior tendon dysfunction and attenuation of the spring ligament allow talar head plantarflexion and forefoot abduction. This relationship matters in surgical planning, because reconstruction may combine spring ligament repair or augmentation with a calcaneal osteotomy, tendon transfer, or subtalar fusion depending on deformity stage. Small structure, big consequences.
