Description
Seen from a posterolateral angle, the gastrocnemius (medial and lateral heads) and the deeper soleus taper inferiorly into the calcaneal (Achilles) tendon, which descends centrally toward its insertion on the posterior calcaneus at the heel. Lateral to the tendon, the fibularis (peroneal) muscle mass and its tendons track behind the lateral malleolus, while the hamstrings and gluteus maximus frame the proximal posterior thigh and hip. Subcutaneous contouring suggests the tendinous narrowing just superior to the ankle joint, with the tendon lying superficial to the deep posterior compartment and posterior to the distal tibia and fibula. Clinically, the posterior view is the one you want when teaching Achilles tendinopathy and rupture because it clarifies where thickening, nodularity, and focal defects occur along the mid-portion (typically 2 to 6 cm proximal to the calcaneal insertion). It also maps the relationship between the tendon and the retrocalcaneal region, a frequent pain generator in insertional tendinopathy and retrocalcaneal bursitis, and it supports discussion of the Thompson test and the loss of plantarflexion strength when the gastrocnemius-soleus unit no longer transmits force. Palpation landmarks are obvious. So are the sites surgeons target during open repair or percutaneous techniques, where protecting the sural nerve and paratenon becomes a practical concern.
