Running proximally from the posterior calcaneal tuberosity, the Achilles (calcaneal) tendon forms a thick, glistening cord in the midline of the hindfoot and distal leg. Superiorly, its fibers continue into the gastrocnemius and soleus of the triceps surae, while the tendon sits superficial to Kager’s fat pad and the posterior aspect of the talocrural joint. Medial and lateral borders are flanked by the paratenon and adjacent soft tissues of the ankle, with the calcaneal insertion lying distal to the malleoli. A palpable landmark. Clinically, this posterior view supports teaching of tendon rupture patterns, which often occur 2 to 6 cm proximal to the calcaneal insertion where vascularity is relatively limited and degenerative tendinopathy accumulates. Orientation matters for operative repair and percutaneous techniques, since the sural nerve and small saphenous vein course in the posterolateral ankle and can be injured when incisions drift laterally. The calcaneal insertion also frames differential diagnosis between midsubstance tendinopathy and insertional enthesopathy with retrocalcaneal bursitis. Use it to anchor lower limb gross anatomy labs, sports medicine teaching on Achilles tendinopathy and rupture, and surgical education for open versus percutaneous repair approaches and portal placement.
