Description
Arising as a terminal branch of the popliteal artery in the popliteal fossa, the posterior tibial artery descends in the deep posterior compartment of the leg, coursing inferiorly toward the medial malleolus. In posterior view it lies medial to the Achilles (calcaneal) tendon, while the fibula remains lateral and the tibia forms the medial bony column of the lower leg. Proximally, the femur, knee joint, and posterior aspect of the tibia frame the vascular transition from femoral to popliteal circulation, then into the tibial vessels that accompany their venae comitantes. Red arterial and blue venous channels are shown in relation to the posterior knee and calf. Clear orientation. Palpating the posterior tibial pulse just posterior to the medial malleolus depends on understanding this artery’s medial course and its separation from the Achilles tendon by deep fascia and the flexor compartment. The same relationship underpins common surgical and imaging tasks: Doppler assessment for peripheral arterial disease, mapping inflow for distal bypass, or avoiding iatrogenic injury during medial ankle approaches and tarsal tunnel decompression where the artery runs with the tibial nerve.
