Description

Seen from the medial aspect of the male ankle and hindfoot, the posterior tibial artery descends in the deep posterior compartment of the leg and courses posterior to the medial malleolus toward the tarsal tunnel. It lies adjacent to the tibia and talus proximally and then passes along the medial surface of the calcaneus before dividing distally into the medial and lateral plantar arteries. Superficial and deep venous companions track with the artery, with the vessel network extending into the plantar midfoot along the metatarsals. Orientation matters. That relationship to the medial malleolus is a daily clinical landmark because the posterior tibial pulse is palpated just posterior and inferior to the malleolus, and diminished flow here raises concern for peripheral arterial disease in patients with diabetes or critical limb ischemia. Surgeons also care about the confined anatomy of the tarsal tunnel, where swelling, scarring, or iatrogenic injury can compromise the posterior tibial artery during fixation of medial malleolar fractures or during tarsal tunnel release; the artery’s bifurcation into the plantar circulation defines the inflow to the medial and lateral plantar angiosomes. Knowing where the vessel turns and divides helps you plan incisions and avoid avoidable bleeding.
