Arising from the popliteal artery within the popliteal fossa, the superior medial genicular artery courses medially and slightly superiorly, wrapping toward the medial femoral condyle just proximal to the knee joint line. From a posterior perspective, the vessel tracks deep to the superficial soft tissues and approaches the posterior aspect of the distal femur before contributing to the periarticular genicular anastomosis. The distal femur and proximal tibia frame the field, with the patella positioned anterior to the joint and largely serving as an orientation landmark rather than the focus. A single contrasting vessel may indicate an adjacent vein, reinforcing the layered vascular relationships behind the knee. Posterior knee vasculature is a frequent source of avoidable injury. The superior medial genicular artery sits in the corridor encountered during posterior and posteromedial approaches to the knee, and its proximity to the medial femoral condyle matters when placing retractors, repairing the posterior capsule, or working near the medial head of gastrocnemius. Genicular branches can also become clinically visible in collateral patterns around chronic femoropopliteal occlusive disease, and they are relevant during angiography, embolization planning, and operative control of bleeding after trauma. Small vessel, big consequences. Use this plate when teaching knee regional anatomy, the popliteal fossa, or periarticular arterial networks in gross anatomy, orthopaedics, sports medicine, or vascular surgery curricula.
