Description

Arising from the medial aspect of the knee, the superior medial genicular vein courses posteriorly along the femoral condyle, tracking close to its companion arterial branches before joining larger venous channels in the popliteal fossa. Proximally it lies posterior to the distal femur and deep to the posterior capsule, while distally it approaches the superior margin of the tibial plateau and the medial head of gastrocnemius. The popliteal vein occupies a more central and slightly lateral position within the popliteal space, receiving genicular tributaries that skirt the joint line. Small venules around the periarticular plexus communicate across the medial and posterior compartments. Posterior knee venous anatomy matters when the popliteal space becomes a corridor for intervention. Arthroscopic portals, posterior capsular releases, and open approaches for PCL avulsion, meniscal root work, or synovectomy can all encroach on the genicular vessels if the surgeon underestimates how closely they hug the posterior condyles. Bleeding from a superior medial genicular branch is usually controllable, but in a tight popliteal compartment it obscures landmarks quickly. Venous patterns also help interpret postoperative swelling and duplex ultrasound in suspected deep venous thrombosis, where distinguishing small genicular tributaries from the popliteal vein reduces false positives.
