Description
Arising from the medial aspect of the perigenicular venous network, the inferior medial genicular vein tracks along the posteromedial knee, coursing near the medial tibial condyle and the posterior horn region of the medial meniscus. Adjacent bony landmarks include the distal femur, proximal tibia, and their articular cartilage surfaces, with the vein positioned superficial to the joint capsule as it converges toward larger popliteal venous channels. Posteriorly, the popliteal artery and popliteal vein occupy the central fossa, while the tibial nerve lies close by, typically the most superficial structure in the popliteal fossa, with the common fibular (peroneal) nerve angling laterally toward the fibular neck. Small venules and communicating vessels reinforce the medial genicular drainage pattern. A small vessel, but a consistent landmark. This posteromedial emphasis matters when teaching or planning around the medial compartment and posterior capsule, where vascular structures can be a source of bleeding or postoperative hemarthrosis even when the main popliteal vessels are protected. Arthroscopic work on the posterior horn of the medial meniscus, posteromedial portal placement, and inside-out meniscal repair all bring instruments and suture needles into a corridor where genicular veins and their companions may be tethered by synovium and capsular tissue. Venous anatomy here also helps explain the spread of periarticular infection or inflammatory effusion into the popliteal region and can be correlated with Doppler evaluation when pain localizes medially but deep venous thrombosis is in the differential.
