Running along the anteromedial aspect of the distal femur, the superior medial genicular vein is traced as it courses deep to the vastus medialis and approaches the medial femoral condyle. Small venules converge near the medial retinacular tissues before the vein curves posteriorly toward the perigenicular venous network and the popliteal vein. Superior lies the femoral shaft, inferior the tibial plateau with the medial meniscus interposed, and laterally the fibular head sits outside the primary field of venous drainage. Blue vascular channels encircle the joint line. Anterior orientation at the knee matters because the peripatellar anastomoses are where surgical planes and bleeding risks often collide, especially during medial parapatellar arthrotomy for total knee arthroplasty and during exposure for medial meniscus repair. The superior medial genicular vein sits close to the medial superior genicular artery, and confusing the two in dissection or illustration leads to poor teaching and sloppy operative planning. Expect traction or cautery in this region to affect venous outflow and contribute to postoperative hematoma around the medial gutter, a common nuisance after knee arthroplasty and synovectomy.
