Description
Arising from the posterolateral periarticular plexus of the knee, the superior lateral genicular vein courses medialward along the lateral femoral condyle, deep to the biceps femoris tendon and adjacent to the capsule. Proximally it converges toward the popliteal fossa, joining the venous channels that form the popliteal vein posterior to the distal femur. Distal to the joint line, smaller venules from the synovium and posterior capsule feed into this vessel while the tibial plateau and articular cartilage frame the joint surfaces inferiorly. Orientation is strictly posterior, so the vessel relationships are read against the intercondylar region and posterior aspects of the femoral and tibial condyles. Posterior genicular venous anatomy matters when you are working around the popliteal fossa, where the popliteal vein lies superficial to the popliteal artery and is vulnerable during posterior knee approaches. Varicosities and postsurgical bleeding can track along the genicular veins, and their communications with the popliteal system help explain calf swelling patterns in deep venous thrombosis and post-arthroplasty hematoma. Small vessel, big consequence. This view also supports teaching of the genicular anastomosis concept, because the lateral genicular vessels form a recognizable periarticular ring that mirrors the arterial network.
