Description
Posterior to the knee joint, the superior medial genicular vein tracks along the superomedial margin of the femoral condyle, coursing deep to the skin and subcutaneous fascia toward the medial aspect of the popliteal fossa. Proximally it communicates with tributaries draining the distal thigh, while distally it receives small venules from the medial capsule and periarticular soft tissues. Medial to lateral, its relationship is read against the popliteal vein and its genicular contributors, with the vessel set just superior to the joint line. Small but consistent. Because this vein participates in the periarticular genicular venous network, it becomes relevant any time the posterior knee is approached or manipulated, including arthroscopy portal placement, posterior capsular work, and open procedures around the medial femoral condyle. Varicosities and venous reflux in the great saphenous system can recruit genicular tributaries, and the superior medial genicular vein may be encountered as a bleeding source during medial knee incisions or dissection through the posteromedial corner. The posterior orientation also helps clarify how venous return from the medial knee converges toward the popliteal vein before continuing proximally into the femoral venous system.
