Running along the medial aspect of the distal thigh, the superior medial genicular artery branches from the popliteal artery and courses anteriorly above the medial femoral condyle toward the periarticular anastomosis of the knee. In an anterior presentation, its path sits deep to the sartorius and vastus medialis region, crossing toward the medial patellar retinaculum and joining contributions from the descending genicular and inferior medial genicular arteries. Red arterial channels track distally toward the anterior tibial circulation, while blue venous companions outline the superficial and deep drainage, including venous plexuses concentrated around the patella and medial joint line. The superior medial genicular artery is a frequent contributor to periarticular collateral flow when the superficial femoral or popliteal artery is stenosed, and it is also a vessel at risk during medial arthrotomy, total knee arthroplasty exposure, and medial retinacular releases, where uncontrolled bleeding can obscure the field and hematoma can threaten wound healing. Sports medicine and trauma contexts also matter: periarticular genicular branches can be injured with distal femur fractures or severe valgus stress, and they are common targets in selective angiography or embolization for recurrent hemarthrosis after knee surgery. This relationship to the medial femoral condyle makes orientation straightforward. Useful for teaching lower limb vascular anatomy in gross anatomy and surgical anatomy courses, and for illustrating periarticular genicular anastomoses in orthopedic, sports medicine, and interventional radiology publications.
