Description
Posterior to the knee joint, the popliteal artery descends in the popliteal fossa and gives off the inferior medial genicular artery, which courses medially and anteriorly along the proximal tibia toward the medial tibial condyle. Its small-caliber branches track close to the joint capsule and the medial collateral ligament region, forming an anastomotic network with adjacent genicular vessels. Deep to the superficial fascia, the vessel sits near the popliteus and the posterior horn of the medial meniscus as it wraps around the posteromedial corner of the joint. Orientation matters. Clinical teaching often overlooks the inferior medial genicular artery until you meet it in the operating room. During medial knee arthrotomy, meniscal repair, high tibial osteotomy, or hamstring tendon harvest for ACL reconstruction, this artery and its accompanying venae comitantes can be a source of persistent posteromedial bleeding, and injury to its branches has been discussed in relation to vascular compromise of the medial meniscus and adjacent capsuloligamentous tissues. A posterior perspective also clarifies why genicular anastomoses can maintain distal perfusion when the popliteal artery is stenosed or temporarily clamped, yet still leave patients at risk for ischemic pain with exertion in popliteal artery entrapment patterns. Use this illustration in gross anatomy and musculoskeletal anatomy labs to anchor the popliteal fossa contents and the genicular arterial ring, and in orthopedic surgery or sports medicine teaching files when describing posteromedial approaches and arthroscopic portal safety. It also fits well in clinical vascular modules covering collateral circulation around the knee and interpretation of posterior knee angiography.
