Description

Arising from the popliteal artery in the posterior knee and curving toward the anterior compartment, the inferior lateral genicular artery tracks laterally around the proximal tibia just inferior to the lateral tibial condyle. From an anterior perspective, it courses deep to the lateral retinaculum and adjacent extensor apparatus, approaching the lateral margin of the patella to contribute to the peripatellar anastomotic network. The lateral femoral condyle sits superior to the vessel’s path, while the tibial plateau and proximal fibula lie inferior and lateral as bony landmarks. Small companion veins parallel the arterial route. A small vessel, big relevance. This view matters because the inferior lateral genicular artery is one of the consistent contributors to patellar and anterolateral knee perfusion, and it sits in the corridor used for common lateral approaches and arthroscopy portal placement. Iatrogenic injury can complicate procedures ranging from lateral meniscal repair to tibial plateau fixation, where retraction near the anterolateral proximal tibia risks bleeding and postoperative hemarthrosis. The anastomosis with the superior lateral genicular, anterior tibial recurrent, and descending branch of the lateral circumflex femoral artery also explains why segmental compromise may be clinically silent in some patients but consequential in others with peripheral arterial disease or prior surgery.
