Posterior perspective of the Inferior Lateral Genicular Artery

Description

Posterior to the knee joint, the popliteal artery descends in the popliteal fossa and gives rise to the inferior lateral genicular artery just proximal to the tibial plateau. From its origin on the lateral aspect of the popliteal trunk, the vessel sweeps inferolaterally, coursing deep to the lateral head of gastrocnemius and toward the fibular (lateral) side of the proximal tibia. Adjacent landmarks in this view include the distal femur, the posterior margins of the medial and lateral femoral condyles, the proximal tibia and fibular head, and the articular cartilage lining the condyles and tibial plateau. A posterior perspective makes the genicular branching pattern easier to teach because it places the popliteal artery, its genicular branches, and the posterior capsule in the same field. Small vessel, big consequence. The inferior lateral genicular artery contributes to the perigenicular anastomosis and becomes clinically relevant when collateral flow is needed after femoropopliteal occlusive disease, and it is also at risk during lateral approaches around the fibular head and posterolateral corner reconstruction where dissection tracks near the capsule and lateral gastrocnemius.
