Description
Running along the lateral aspect of the knee, the superior lateral genicular artery branches from the popliteal artery and courses anteriorly, hugging the distal femur just proximal to the lateral femoral condyle. From this perspective it passes deep to the tendon of vastus lateralis and related fibers of the quadriceps femoris, then contributes to the periarticular genicular anastomosis around the patella. The lateral tibial condyle, fibular head region, and the patella positioned anterior to the femoral trochlea frame the vessel’s trajectory, while the lateral collateral ligament (fibular collateral ligament) sits more superficial and posterior relative to the anterior sweep of the artery. Smaller accompanying veins and arteriolar branches track alongside the main vessel. Small but clinically noisy. Lateral genicular vessels matter when you work around the lateral retinaculum and distal femur because they are a common source of unexpected bleeding in arthroscopy portals, lateral release, and open approaches for distal femoral osteotomy or fracture fixation. Their relationship to the vastus lateralis tendon helps explain why traction, aggressive cautery, or deep dissection near the lateral suprapatellar pouch can compromise peri-patellar blood supply and contribute to hemarthrosis. This course also underpins teaching of the genicular anastomosis, a collateral network that can maintain distal limb perfusion when the superficial femoral or popliteal artery is stenosed. Orthopedic surgery texts, sports medicine lectures, and arthroscopy training modules use this view to orient learners to the vascular “no surprise” zones on the lateral knee. It also fits anatomy lab manuals and radiology correlations when discussing periarticular vascularity and postoperative hematoma risk after lateral approaches to the distal femur.
