Seen from the posterior aspect of the ankle, the posterior tibiotalar part of the medial ligament (deltoid ligament) spans from the posterior colliculus of the medial malleolus of the tibia to the posteromedial talus near the posterior tubercle. Medial structures lie to the left of the mortise and the fibula remains lateral, framing the talus between the malleoli as the ligament fibers course inferiorly and slightly laterally across the back of the joint. Bony landmarks you would expect include the tibial plafond superiorly, the talar trochlea centrally, and the posterior process of the talus immediately deep to the ligament. A tight posterior band. This posterior view matters because the posterior tibiotalar fibers act as a restraint to posterior talar translation and external rotation of the talus within the mortise, stabilizing the hindfoot when the ankle is loaded and the subtalar joint contributes to complex rotation.
