Description
Positioned in lateral profile, the urinary bladder occupies the anterior pelvis, with the prostate gland immediately inferior to the bladder neck and the proximal urethra coursing inferiorly from the internal urethral orifice. Posterior to the bladder base, the rectum and its surrounding rectal venous plexus form a flattened, branching network along the rectal wall, continuous superiorly toward the superior rectal vessels and inferiorly toward the anal canal region. Fine neural elements track along the posterolateral bladder and toward the prostate, consistent with pelvic autonomic pathways that run in close association with the pelvic fascia. Spatially, the venous net hugs the rectum posterior to the prostate and posteroinferior to the bladder. Lateral anatomy of the rectal plexus matters because this is where venous drainage patterns intersect with common disease and operative planes: internal hemorrhoids represent dilatation of the internal rectal venous plexus above the pectinate line, while pelvic surgery can injure adjacent autonomic fibers that modulate bladder and erectile function. A critical landmark. Surgeons working in the total mesorectal excision plane, or performing radical prostatectomy, must respect the proximity of the rectum, periprostatic fascia, and venous plexuses to limit bleeding and preserve pelvic splanchnic nerve function.
