Description
From a superior perspective, the atlas (C1) sits centrally as a ring-shaped vertebra with prominent lateral masses and a broad posterior arch, positioned immediately inferior to the occipital condyles and superior to the axis (C2). Laterally, the transverse processes project toward the first ribs, and the transverse foramina are visible on either side where the vertebral arteries pass. The upper thoracic aperture frames the vertebral column, with the posterior ribs curving anterolaterally from their costovertebral region relative to the midline cervical spine. Alignment between the cervical vertebrae and the rib ring is emphasized. This angle matters when teaching atlanto-occipital and atlanto-axial biomechanics, where small changes in joint congruence produce large changes in head posture. The relationship of the transverse foramen to the lateral masses provides a direct anatomical rationale for why posterior C1 instrumentation and lateral mass screw trajectories demand careful planning, and why vertebral artery injury remains a feared complication.
