Description
Arising from the abdominal aorta just inferior to the aortic hiatus, the right inferior phrenic artery courses superolaterally toward the central tendon of the diaphragm and the bare area of the liver. From an inferior (subdiaphragmatic) perspective, its proximal segment lies in close proximity to the caval opening, with the inferior vena cava positioned slightly to the right of midline and anterior to the vertebral column. Small branches typically fan medially toward the esophageal hiatus and laterally along the diaphragmatic undersurface, while the phrenic vein counterparts track nearby. Tight anatomy. For procedural planning, the inferior view matters because the right inferior phrenic artery is a frequent extrahepatic feeder to hepatocellular carcinoma, often supplying lesions abutting the diaphragm and hepatic dome, and it can be targeted during transarterial chemoembolization when hepatic arterial supply proves incomplete. Its relationship to the inferior vena cava orifice also helps explain why catheterization and embolization demand careful angiographic orientation, as reflux or non-target embolization can threaten diaphragmatic perfusion and irritate the phrenic nerve, leading to post-procedural shoulder pain and hiccups. Collateral pathways to the suprarenal gland and esophagus are common teaching points in this region. Radiology and interventional texts can pair this plate with angiographic schematics to clarify the origin, course, and likely anastomoses of the inferior phrenic artery in the upper abdomen. It also fits well in gross anatomy labs and surgical anatomy courses when discussing the diaphragm’s arterial supply, the caval aperture, and approaches near the hepatic bare area during mobilization of the right lobe.
