Description
Arising from the conus arteriosus of the right ventricle, the pulmonary trunk (truncus pulmonalis) is shown in anterior section at its root (radix trunci pulmonalis) as it courses superiorly and slightly leftward from the pulmonary valve. The anterior ventricular wall has been cut away to expose the outflow tract, placing the semilunar cusps and the sinuses of the pulmonary valve at the junction between ventricular myocardium and the elastic wall of the great vessel. Immediately posterior and slightly to the right, the ascending aorta lies in close apposition, while the infundibulum forms the muscular funnel proximal to the arterial root. This cut is the one you want when teaching or planning around right ventricular outflow tract (RVOT) pathology, because it clarifies where muscle ends and the arterial root begins, and how little tissue separates the pulmonary trunk from the aortic root at their bases. Surgeons rely on these relationships during pulmonary valve-sparing repairs and RVOT reconstruction (for example after tetralogy of Fallot repair), where an incision placed too far proximally risks injury to the valve leaflets and too far distally compromises the arterial wall and commissural support. Clear orientation to the anterior aspect also helps explain why congenital or post-surgical pulmonary regurgitation produces progressive right ventricular dilation, starting at the outflow tract.
