Description
Anterior deep sectional anatomy centers on the pulmonary trunk (truncus pulmonalis) opened to expose the pulmonary valve and the three sinuses of the pulmonary trunk (sinus trunci pulmonalis) positioned immediately superior to each semilunar cusp. Each sinus forms a pocket-like expansion of the arterial wall at the root, separated from its neighbors by the commissures where adjacent cusps meet. Superiorly the sinuses blend into the tubular lumen of the pulmonary trunk, while inferiorly the cusp attachments mark the level of the ventriculoarterial junction as the right ventricular outflow tract transitions into the arterial root. Clinical relevance sits at the pulmonary root. Congenital pulmonary valve stenosis and dysplastic semilunar cusps alter sinus geometry, which affects cusp coaptation and can produce pulmonary regurgitation after balloon valvuloplasty or surgical valvotomy. For Ross procedures, homograft implantation, and complex right ventricular outflow tract reconstruction, understanding where the sinuses end and the sinotubular junction begins helps the operator size the root and avoid distortion of the commissures.
