Description

Seen in lateral profile of the male head and neck, the semispinalis capitis is traced as a deep, vertically oriented component of the transversospinalis group, running superiorly from cervical and upper thoracic transverse processes toward its insertion on the occipital bone between the nuchal lines. Superficial to it, the sternocleidomastoid forms an oblique strap from the mastoid process to the sternum and clavicle, while posteriorly the upper trapezius and splenius capitis define the posterior triangle contours. Anteriorly, the masseter at the mandibular ramus and the temporalis at the temporal fossa frame the zygomatic arch and temporomandibular region, with the auricle providing a fixed landmark just lateral to the mastoid. Semispinalis capitis matters because its fiber direction and deep position explain both movement and symptoms, extending the head and contributing to contralateral rotation while lying in close relationship to the greater occipital nerve as it courses toward the scalp. Tender points or overuse after whiplash can refer pain to the occiput and vertex, a common pattern in cervicogenic headache. For posterior cervical exposure, this view also helps learners keep straight which muscles must be reflected to reach the suboccipital region without confusing semispinalis capitis with the more superficial splenius capitis.
