Description
Posteriorly, the illustration isolates the semispinalis capitis on the lateral aspect of the upper cervical region in an adult male, set against the occipital bone and the upper cervical to upper thoracic vertebrae. The muscle belly lies deep to the more superficial posterior neck layers (trapezius and splenius capitis would normally overlie it) and runs superomedially toward its insertion on the occipitalis, between the superior and inferior nuchal lines. Inferiorly, its fascicles arise from the transverse processes of the upper thoracic vertebrae and the articular processes of the lower cervical vertebrae, spanning multiple motion segments. Bony landmarks include the midline spinous processes, the posterior elements of the atlas and axis, and the medial borders of both scapulae laterally. Semispinalis capitis is a primary head extensor and a strong contralateral rotator of the head and upper cervical spine when acting unilaterally. Clinically, this deep extensor group often becomes a pain generator in cervicogenic headache and chronic neck pain, where myofascial trigger points can refer pain to the occiput and vertex, mimicking occipital neuralgia. It also matters in posterior cervical approaches, because dissection through the posterior midline and subperiosteal elevation can denervate or atrophy deep extensors, contributing to postoperative axial neck pain.
