Description
Seen in lateral profile on an adult male neck, the semispinalis capitis occupies the deep posterior compartment, running superomedially from the cervical articular and transverse processes toward the occipital bone between the superior and inferior nuchal lines. Anterior to it, the sternocleidomastoid forms the dominant superficial strap from the manubrium and medial clavicle to the mastoid process, creating a clear superficial to deep contrast across the lateral neck. Posterior elements of the cervical vertebrae, including spinous processes and intervertebral discs, provide bony reference for the muscle’s segmental origins, while the auricle and mastoid region anchor surface anatomy. Depth is the point. Because semispinalis capitis is a prime extensor of the head and a strong contralateral rotator, its deep fiber direction is a reliable teaching target when explaining coupled motion in the upper cervical spine (C0 to C2) versus the mid cervical segments. Clinically, this muscle frequently contributes to cervicogenic headache and post whiplash pain patterns, with trigger points referring toward the occiput and behind the ear, and it is often palpated just posterior to the sternocleidomastoid when differentiating superficial myofascial pain from facet-mediated symptoms. The lateral view also helps frame posterior cervical approaches, where preservation and retraction of deep extensors influence postoperative neck pain and extensor strength.
